Eligibility
REQUIREMENTS
In order to be eligible to receive assistance from CC5, you must meet the following requirements:
1) Juniata County Residency
You must be a resident of Juniata County and provide proof of that residency
2) Employment
You must be able to provide proof of current, employment income in the form of a pay stub.
Elderly/retired individuals can provide proof of retirement income in the form of pay stub or other payment receipt.
If returning to the workforce and applying for return-to-work assistance, you must provide employment offer letter, with start date.
Individuals without employment are ineligible and are encouraged to seek help from local, public assistance.
3) Public Assistance
Applicants that are currently receiving or are likely eligible for public assistance benefits such as SNAP, TANF, Medical Assistance or LIHEAP are not eligible to receive assistance benefits from Community Cares 5 Dollar Fund.
Retirees receiving Medicaid-only public assistance are NOT excluded from eligibility.
4) "Pay-It-Forward" Eligible
CC5 is an organization based on the good faith and generosity of others. Individuals that receive assistance will be asked to return the favor. All beneficiaries will sign a "pay it forward" clause where they agree that they will respect those that helped them by returning $5 to Community Cares 5 Dollar Fund in order to assist another individual.
Parties that do not fulfill this requirement within 3 months of receiving assistance will be excluded from eligibility to receive future CC5 benefits.
5) All households are limited to receiving any form of assistance once within a calendar year.
Disclosure:
All assistance is subject to Board approval and availability of funds.
As a publicly funded organization with accountability to the community, if during the application verification process an individual is identified as having prior criminal charges, the Board has the right to refuse assistance.
